Our practice offers a wide range of payment options. Financing with no initial payment may often be arranged. We will work out a plan suited for your individual needs. For those who pay in full at the beginning of treatment we offer a discount.

Insurance
We will be happy to assist you in processing insurance claims. If you are unaware of your benefits, we will be glad to help you if you bring your policy manual with you. Your company may require that a treatment plan be submitted in advance to your carrier for a predetermination (authorization) of benefits.

After the initial examination and diagnosis, we will be glad to prepare and submit the necessary forms. In order to do this, please bring your insurance forms with you to the first appointment. For our Temporomandibular Joint Dysfunction patients, please bring Major Medical Insurance forms as well as Dental Insurance forms.

Some health insurance programs provide limited coverage, some have age limitations, and some provide no coverage. We suggest you become fully aware of the provisions of your policy since we are a third party to your insurance agreement.

Our fee will vary according to the type of medical and dental insurance information. A portion of your fee may be covered by one or both of these insurance policies.

If you have been treated previously by another practitioner, it would be helpful if you would bring any appliances and records which are in your possession.
